Primary care doctors and nurse practitioners often struggle to find services a patient needs. It adds to provider cost and frustration while reducing the patient’s options, access and quality of care. This is a solution to aid navigation and explore what’s possible in clinic to clinic support structures.

Risk Mitigation for the Unattached Patient
Unattached patients are growing in number. This means more individuals are subjected to the barriers to care and lagging access to health interventions such as cancer screening and vaccinations. This project addresses these gaps through co-design with patients with a focus on health seeking behaviour and supporting the activated patient.
We regularly run workshops to improve our understanding of user needs and gather important feedback. All participants are paid for their time. We are currently recruiting patients for upcoming workshops in Toronto.

Our thesis is that good healthcare is based on solid foundations.

About
Building Block Health is led by Ilan Shahin MD, MBA, CCFP is a family doctor working in emergency medicine with experience in urban and rural communities. He was a co-founder of a digital health company that made several breakthroughs in Ontario in referral management, wait time reporting, and navigation support to primary care offices across the province.
